CAUTION: This calculator should NOT be used to determine doses when converting a patient from one opioid to another. This is especially important for fentanyl and methadone conversions. Equianalgesic dose ratios are only approximations and do not account for genetic factors, incomplete cross-tolerance, and pharmacokinetics.
The AMDG Guideline recommends not exceeding 120mg/day MED without first obtaining a consultation from a trained pain specialist who agrees that a higher dose is indicated and appropriate. This opioid dose calculator was developed by the Washington State Agency Medical Directors' Group to be used in conjunction with the Interagency Guideline on Prescribing Opioids for Pain. † Buprenorphine has partial µ-opioid receptor agonist activity with resultant ceiling effects compared with full µ-opioid receptor agonists. Buprenorphine transdermal is NOT approved for opioid use disorder, and its inclusion in this calculator should not be applied to clinical decisions related to the management of opioid use disorder.
